16 yr female with continuous pain in leg since 5 months following h/o fall.. Possibilities....?

  • Ahmed Hashhash MRI........Brodies abscess????
  • Jayant Sharma Specify the location of pain. Relation to movement. And any other constitutional symptim. Skin condition, swelling etc. Range of movts are important to not. Keep in mind contusion to quadriceps. Sonk.
  • Jayant Sharma Sorry not sonk age is not in favour
  • Rajasekhar Rao P giant cell tumor
  • Ahmed Hashhash Giant cell tumour is epiphyseal lesion and adult aged.......
  • Rajasekhar Rao P here epiphysis is closed I dont see it
  • Ahmed Hashhash here localized metaphyseal well defined witrh surrounding marginal sclerosis.........never to be gct...
  • Jayant Sharma Dr Rao, sorry for my ignorance, where do you see this GCT tibia or femur. Epiphyseal closure happens at 18 and around in knee, if i am not wrong.
  • Vinay Kandukuri Fall may be incidental, there us clear cut osteolytic lesion in metadiaphyseal junction with sclerotic margin guess mri will answer more questions
    My guess is ossifying fibroma
  • Abdullrab Almarwanya Thank you for presenting this case : The x ray shows marked sclerosis of the metaphyseal diaphyseal region of the proximal tibia obliterating the normal marrow shadow with some area of sclerosis surrounding variably sized, lytic lesion with no cortical destruction or soft tissue extension which could correlate more with a chronic inflammatory process(Brodie's abcess) , the history is the first step to look for other constitutional symptoms or draining sinus , then CBC (WBC),ESR ,CRP but they are often lacking.Biopsy :aspiration for histopathology ,culture &sensitivity are necessary.
